Shirley Ann Mack Cook, 80, of Collins and daughter of James D and Bertie Lee Mack, died early Wednesday, August 24, 2016 at Azalea Health and Rehabilitation in Metter.
Ms. Cook was born and lived most of her life in Collins and was a retired Nursing Assistant having worked for several nursing home facilities in the Toombs, Bulloch and Evans County areas. She was a member of the Collins First Baptist Church. In addition to her parents, Ms. Cook was preceded in death by two brothers, Franklin Mack and Elliot Mack and one sister, Ava Nell Watts.
Surviving are a sister, Alene Paul of Pooler; two sister-in-laws, Carolyn Mack of Collins and Lois Mack of Jacksonville, FL; and several nieces and nephews.
The funeral will be held 11 o’clock, Friday, August 26 at Collins First Baptist Church with Rev. David Lamb officiating.
Visitation will be held at the Collins First Baptist Church, Friday, August 26 from 10 o’clock until the funeral hour.
Interment will be in Collins First Baptist Church Cemetery.
Kennedy Funeral Homes, Cobbtown Chapel is in charge of the arrangements.
